What is Family and Community Health?
For families and communities to stay healthy, they need access to primary health care providers, like family physicians and nurse practitioners. In many communities, there are not enough physicians, which can make it difficult for families to reach services that are important for the prevention and treatment of common health conditions. Family and community health programs try to improve families’ well-being by improving access to physicians, nurses and other health-care providers.

UHN’s Family and Community Health Program
Based at Toronto Western Hospital, our program links with the surrounding community to provide primary care to families and community members, with special attention to the needs of the aging population, children and women.

With the teamwork of pharmacists, social workers, physicians, primary care nurse practitioners and staff nurses, we do our best to help families deal with a broad range of health concerns. We also help connect families with the care of specialists when needed.

Better health — within families’ grasp
To make it easier for families to reach health care services, we have built the following features into our program:

  • Extended hours including evenings and weekends at Toronto Western Hospital
    • Mondays - 9:00 a.m to 8:00 p.m.
    • Tuesdays - 9:00 a.m to 8:00 p.m.
    • Wednesdays - 1:30 p.m. to 8:00 p.m.
    • Thursdays - 9:00 a.m to 8:00 p.m.
    • Fridays - 8:30 a.m. to 4:30 p.m.
    • Saturdays - 9:00 a.m. to noon
  • HEALTHSTYLES - A sexual health clinic for men and women, providing birth control methods, relationship counselling, emergency contraception, pregnancy counselling, sexually transmitted infection screening and treatment.
    • Wednesdays - 1:30 p.m. to 6:30 p.m.

  • Family practice inpatient beds on medical unit
  • Medical support for addictions through the Mental Health and Addictions mental health services
