The Community Interlink Program offers the following services to the Toronto area:
- Psychosocial support and caring for patients with cancer and their families from diagnosis through cure or dealing with an incurable disease, death and dying and the grieving process
- Education around symptom management to increase comfort and independence
- Counseling and support to encourage self-reliance
- Linking to other healthcare professionals and caregivers when needed
- Referrals and information on community resources
- Assisting with bridging communication and information gaps to help patients make informed decisions.
**NOTE: Any patient with cancer or their family member residing in the City of Toronto can be referred to the program. You do not have to be a patient at Princess Margaret Hospital.
