The InfoWell Kidney Portal is part of the InfoWell website providing health information specific to those with chronic kidney disease. This is a secure website created by Shared Information Management Services (SIMS) in partnership with health care professionals and patients.
The Portal provides health information and educational tools 24 hours a day, 7 days a week. It can help you to better understand and manage your disease.
The InfoWell Kidney Portal provides patients with printable tools to create:
