Cardiac Care and Heart Health
Our program helps patients achieve and maintain optimum cardiac health, with a team that includes nurse practitioners, a dietitian, nurse, psychologist, pharmacist, exercise specialists, a social worker and doctors.
We help patients develop strategies to reduce or eliminate their risk of coronary artery disease; prevent or minimize hospitalization; decrease mortality; and improve quality of life.
Our services:
- Ambulatory cardiac health care and education for patients with cardiac problems
- Education, with risk reduction interventions for individuals with coronary artery disease or those at risk for coronary artery disease
- Exercise reconditioning and education for patients in the acute post-cardiac event phase
