Geriatric Clinic

The Geriatric Program at UHN is part of a formal regional geriatric program (RGP) that covers all of Toronto, to ensure that a full spectrum of care for frail elderly patients exists throughout the city. UHN (Toronto General and Toronto Western Hospitals) and Toronto Rehabilitation Institute work together as part of a coordinated delivery system.

• Day hospital and multidisciplinary clinics: Toronto Rehabilitation Institute
• Joint outreach consultation service provided by UHN and Toronto Rehab (multidisciplinary geriatric consultations in patients' homes)
• Geriatric Rehab inpatient unit: Toronto Rehabilitation Institute
