We work in the community with people who have severe and persistent mental illness and therefore require a high degree of personalized help and contact with community health professionals who can provide one-on-one attention when needed.

We can:

| • | Work with the client to develop a customized, individualized plan for living in the community. |
| • | Help develop vocational, interpersonal and social relationships by encouraging participation in community activities. |
| • | Monitor, adjust and deliver medication to the client. |
| • | Help the client develop living skills. |
| • | Advocate on behalf of the client |
