The Centre for Admitting, Registration and Discharge Services (CARDS) is a coordinated point of entry for patients being registered and admitted to the Toronto Western Hospital.
Patient Registration
Are you new to University Health Network? If you are a new patient, you need to register with the Admitting department at one of the three hospitals (Toronto Western Hospital, Toronto General Hospital, and Princess Margaret Hospital). This process creates your patient medical record within the hospital's computer system.
Keep in mind:
- You need to register before your medical appointment.
- You only need to register once at University Health Network. Registration at any one of the hospitals (Toronto Western Hospital, Toronto General Hospital and Princess Margaret Hospital) is good for all three hospitals. After registering, you will receive a blue hospital card that can be used at all three hospitals.
- You need to bring your blue hospital card to all of your appointments and tests.
- You only need to renew your hospital card if you have changed your personal information (i.e. you moved and have a new address).
Information Desk
- Staff at the information desk direct patients to their appointments within the hospital. They also provide information and assist visitors.
- Volunteers help direct patients and escort patients to their appointments.
Admitting
There are three ways patients are admitted to the hospital:
- Elective - A pre-scheduled surgery coordinated between the patients' doctor, the hospital and the patient.
- Urgent Admission - An unexpected admission which precludes a doctor's appointment.
- Transfer - When a patient moves from one facility to another (i.e. from one hospital to another hospital or from a nursing home to a hospital).
