SAMPLE LETTER
Dear ,
Recently several clinical research studies at University Health Network were reviewed. Our reviews brought to light concerns about how well the research data in some studies had been documented. One of the studies was a clinical trial in which you took part at Princess Margaret Hospital.
We found errors and discrepancies in the research data that may prevent it from being used for research purposes. Your medical chart has been reviewed, and we are confident that your medical care was not compromised in any way.
University Health Network is committed to preventing such an event in the future. We are developing stronger processes and controls to oversee our clinical research projects and we are certain that these initiatives will ensure the excellence of future research studies across UHN.
We know that you participated in a clinical trial to help us to find new and better ways to treat cancer. We could not conduct research without the support and trust of our patients and we are sorry that, in this instance, we have not lived up to your expectations. Your doctor will be happy to discuss this with you in detail at your next visit. If you would like more information immediately, please go to www.uhn.ca and click on the Princess Margaret Hospital button. Information will be found at “What’s New at PMH” on the right hand side of the page.
Very sincerely,
Dr. Catherine Zahn
