At University Health Network, we recognize that the health information of our patients deserves to be treated with respect and sensitivity and our patients' privacy must be protected. Access to your personal health information is available to those who need to know to provide care, including: physicians, nurses, technicians, therapists and other health professionals. Furthermore, when it comes to collecting and using your personal health information, we take measures to ensure the privacy and confidentiality of the information is protected.

Health Information Protection Act, 2004
  The Health Information Protection Act establishes rules concerning the collection, use and disclosure of personal health information by health information custodians and other persons.
Information and Privacy Commissioner - Ontario
  The Health Information Protection Act designates the Office of the Information & Privacy Commissioner/Ontario (IPC) as the body responsible for overseeing compliance with the provisions of the law. It also gives individuals the right to enquire about health privacy practices covered by the Act.
Canadian Standards Association – Model Code for the Protection of Personal Information
