Healthy Connections was established in 1998 to deliver care that meets the needs of the diverse community served by TWH. Health-care needs in the area are influenced by factors such as a high number of recent immigrants, high levels of unemployment, low levels of household income, low rates of formal education, and a large number of homeless and underhoused.
