This multidisciplinary clinic helps patients and their families cope
with chronic kidney disease and its complications. With the help of
physicians, nurses, a social worker, a pharmacist, and a dietitian, we
provide a variety
of services to patients.

- Consultation with primary nephrologist and primary nurse
- Education about kidney disease and treatment options for end-stage
renal disease
- Nutrition assessment, evaluation and counselling
- Evaluation of current medications
- Social assessment and evaluation
