In preparing for your visit to the Centre for Admitting, Registration
and Discharge Services, please arrive 20 to 25 minutes prior to
your appointment.
What You Need to Bring
- Two pieces of identification — your health card and a second
piece of identification that lists your address.
- The name of your family physician, his/her address and telephone
number.
- The name of your emergency contact, his/her address and telephone
number.
- Your employer's name, address and telephone number.
- Your personal insurance policy/certificate card and credit card
(for a semi-private or private room).
